Awalnya, para Smurf hidup tenteram dan damai di desa tercinta mereka. Tak ada pendatang yang pernah masuk ke desa Smurf sampai Gargamel (Hank Azaria) dan kucingnya berhasil menemukan desa tersembunyi ini. Karena ketakutan, para Smurf pun berusaha melarikan diri dan mencari tempat persembunyian yang aman.

Dalam keadaan panik Clumsy Smurf (Anton Yelchin) masuk ke dalam gua kecil yang sebenarnya adalah gua terlarang. Celakanya, beberapa Smurf lain ternyata malah mengikuti Clumsy Smurf masuk ke gua itu. Sebelumnya Papa Smurf (Jonathan Winters) sempat memperingatkan agar tak ada Smurf yang memasuki gua terlarang ini.

Ternyata, gua ini adalah sebuah portal yang mengantar para Smurf ini masuk ke dunia lain. Tanpa disadari, para Smurf ini terlempar ke New York dan tak bisa mencari jalan untuk pulang. Untungnya, para Smurf ini mendapatkan perlindungan di rumah pasangan Patrick Winslow (Neil Patrick Harris) dan Grace (Jayma Mays). Sambil berlindung, para Smurf ini berusaha mencari jalan untuk pulang ke desa mereka sebelum Gargamel berhasil menemukan mereka.
